New EAP Vendor

As a follow up to the Benefits Flash email below sent on June 7, 2023, the County has now completed the vetting process with Public Risk Innovation Solutions and Management (PRISM) and has determined that CONCERN can provide comparable services to Managed Health Network (MHN) for the Employee Assistance Program (EAP).  Therefore, effective July 1, 2023, EAP services will be provided through CONCERN, click here to view some highlights.  A program plan comparison is also attached, click here to view plan comparison.

If you are seeking services with a provider whom you would like to continue with, an alternate option is to determine if the provider is available through your insurance plan under the life services referral component or if you would like your provider to be part of the CONCERN panel, click here for instructions to start the process.  For employees who are currently working with a counselor, CONCERN will reset the counter and they will be eligible to receive all 5 sessions.

Forms 1095-C and 1095-B

Form 1095-C, Employer-Provided Health Insurance

Under the Affordable Care Act (ACA), Employers are required to report to the Internal Revenue Service (IRS) on Form 1095-C whether the employee participates in an employer offered medical plan. The form provides information on Employee Only about:

• Employees eligibility for employer's coverage.
• Lowest monthly cost plan available to the employee.
• Enrollment in the employer's coverage.

Form 1095-B, Health Coverage

Under the Affordable Care Act (ACA), Health Plans are required to report to the Internal Revenue Service (IRS) on Form 1095-B the employee and dependent names enrolled in an employer sponsored medical plan. Health Plans are also required to provide a copy of Form 1095-B to each employee reported on to the IRS. The health Plan will send the form to the mailing address on file. The form provides information on Employee and Dependents about:

• The employer sponsoring the coverage.
• The coverage provider.
• Information for each covered individual (employee and/or dependents).
